Medicare and Medicaid Fraud

A new report issued by the Government Accountability Office (GAO) has found that the Centers for Medicare and Medicaid Services (CMS) does not adequately enforce Obamacare’s requirement that states fund the cost of state-mandated health insurance benefits.

The report, requested by Energy and Commerce Committee Republicans, raises issues about CMS’s controls to ensure federal taxpayer dollars are not illegally subsidizing state mandates.

Obamacare mandates that insurance plans within the individual health insurance market cover a certain set of essential health benefits (EHBs). States may choose to mandate additional benefits but are then required to cover the cost of such benefits. States are not permitted to use federal tax dollars in the form of Advance Premium Tax Credits (APTCs), to do so.

In its report, GAO found that CMS delegates oversight of this policy to the States instead of enforcing it independently. As outlined in the report:

CMS has limited assurance that APTCs accurately exclude the costs of non-EHB mandated benefits” and that “CMS has expressed concerns about states’ compliance with this requirement,” highlighting critical concerns about the potential for improper federal spending.

“This report, coupled with the immense Obamacare fraud this administration has overseen, highlights the need for Congress and the incoming administration to work swiftly to safeguard taxpayer funds at every corner of the agency. CMS should prioritize rigorous enforcement of all requirements intended to prevent improper spending,” said Committee Chair Cathy McMorris Rodgers (R-WA). “I commend GAO for its investigative efforts, which will help Congress protect taxpayer dollars by correcting government waste, fraud, and abuse.”

Also, why does USGOV force me to pay for Medicare if I don’t want it? Yes, I paid into the fund as a working person because I was forced to, but now that I’m older, I’m also forced to make direct payments under penalty of law. I have health insurance that I buy from my personal funds. I don’t want to pay for Medicare/Obamacare or any other federal medical program. Neither do I wish to subsidize the medical care of illegal aliens. Let them fend for themselves or go home (self-deport).

Shooting handguns and shooting rifles involve different skill sets, and each has its place. I am almost always armed with a handgun, but I’ve used them tactically for my entire adult life. Living in the wilderness, having a point-defense weapon is prudent, but lugging a rifle everywhere is unrealistic. It comes down to tools. My grandfather was an engineer by profession, not a warrior. He admired and used precision rifles. He didn’t get down with other humans in the mud and blood. Had he lived longer, he might have convinced me to go another way – alas. He wasn’t a knife person either, and I find that I have too many knives (yes, I know that’s impossible).

Flouride

For almost 80 years, some American cities have added chemical fluoride to the drinking water for healthy teeth. Since the start, some have claimed it’s an unsafe practice, while others cried that it’s a conspiracy theory. Lisa Fletcher finds a growing number of communities are trying to get the fluoride out and are pointing to sound science.

From homeschooling to home-cooked meals, Kentucky mom Kari Ashauer strongly influences what’s going into her kids’ minds and bodies.

But despite her best efforts, she has no control over something flowing into her home: mandatory fluoride in the tap water.

Kari Ashauer: I don’t get to choose what’s in my water. I have no control over that, especially since it’s a state mandate they must put it in. Kentucky is one of 13 states that require water districts to add fluoride. Across the U.S., about 73 percent of people served by community water systems have fluoridated water.

Since 1951, Kentucky, like many other states around that time, requires fluoride to be added to all tap water. Fluoride was a generally unwanted byproduct of aluminum, fertilizer, and iron ore manufacturing until the industry promoted adding it to water supplies. Today, selling fluoride in water is a $1.2 billion-a-year global business.

(h/t Claudio) Strategypage – December 9, 2024: After nearly three years of war in Ukraine, both sides are relying on cheap, unmanned drones to replace most artillery and aircraft-delivered bombs and missiles. Drones are doing most of the reconnaissance and surveillance as well as confirming how aerial attacks were on or where the enemy currently is. The latest Ukrainian innovation is longer-range drones that can hit targets over 2,000 kilometers away. Using GPS/INS navigation and unjammable terminal pattern recognition guidance, these drones have proved to be formidable aerial weapons. Recently, Ukraine began using longer-range attack drones to hit targets in and around Moscow, Saint Petersburg, Smolensk, and the nearby Crimean Peninsula. Russian air defenses shoot down many of these drones, but many inexpensive drones are being used that some get through and do a lot of damage. When these exploding targets are within sight of a major city, it is difficult for Russian propagandists to claim nothing happened and all the Ukrainian drones were shot down,

While Russia had some long-range bombers at the start of the war, Ukraine soon developed longer-range drones. Russia is running out of its elderly bombers and, because of sanctions, is unable to make any more. It is turning to its remaining stocks of ballistic missiles and cruise missiles for long-range attacks while also building more drones.

Ukraine has long developed and built its drones, and Russia is playing catch up. By early 2023, it was obvious that drones were dominating Ukraine’s battlefield. In July 2024, a senior Ukrainian defense official once more confirmed this and observed that the dominance of drones in combat was increasing. Since the war began in early 2022, drone use has increased at the expense of traditional artillery. The reasons are apparent. Drones are more flexible, cheaper, and come in many different models. Drones can be controlled by nearby operators and sent after targets over a thousand kilometers distant using GPS guidance and several backup systems if electronic jamming of GPS signals is encountered.       1. According to wiki it was designed in New Zealand as short take off for crop dusting and could hold 2000 pounds of chemicals. Then the Australian company bought it and made some changes and produced 138 of them.

        Used in the movie because it looked unusual.

    2. I couldn’t resist, I looked up the PL-12, and the claim is that the barrel shaped fuselage made it easier to refill the fertilizer tank and also protected the fuselage from being bathed in corrosive chemicals.

  3. according to what we were taught (and I remember at this late date), fluorapatite (stronger bonding within the lattice, etc, ad inf) is far more resistant to decay than hydroxyapatite, the primary ~97%, mineral/chemical compound of tooth enamel. newer research since the early ’60s may have controverted this “well-established fact of science”; I wouldn’t know, I only maintained an extremely close acquaintance with the research literature pertaining to the world of endodontics (and that took more than a shovel and a pitchfork).
    However (there always is one of those), fluoride, as I recall, is only effective in the developing fetus, so IMHO why is the general population, other than pregnant women, who should be carefully titrated “with fluoride-containing minerals” by their physicians should both parties consent to such treatment, be exposed‽
    OTOH, not to sound paranoid, do you really know what’s in your AM cup o’ kauphy?

      It’s trying to scare you by saying that it came from an industrial waste byproduct. Anybody who has taken even high school chemistry should know that where it came from doesn’t matter. I don’t know what compound they use in water supplies but the toothpaste dental gels seem to use Sodium Fluoride, so lets say it’s that. All that matters is whether or not the sodium fluoride the water utilities use is chemically purified and they’re not putting literal waste into the water supply. Once it’s chemically pure, the origin means nothing. Every molecule of sodium fluoride is the same as every other molecule of it and not the same as the mess it came from. While I’m no fan of big, intrusive government in any size, it’s hard to imagine the national network of legal scavengers wouldn’t have sued the water treatment places. “Injured by Fluoride? Call Dewey, Cheatem and Howe to get your million dollars!”

      To fall for that is just another indication of how bad public education is.

        1. I’ve heard of the evils of fluoridation for decades. It’s difficult to say from both sides of the argument whether there is a significant enough benefit to offset the risks. The general school of thought is that “the great unwashed don’t brush their teeth with fluoridated toothpaste, so we need to take action and add it to the drinking water to save them.” I added the article because there is about to be a national discussion on the merits of fluoridation. I live in a mountain environment with a well that doesn’t require treatment. It goes down 800 feet and draws from porous limestone. So, it doesn’t apply directly to me. The County tests the water periodically, and I have no problem. I want to know that it’s safe. Sometimes (not near me), arsenic and copper naturally occur and find their way into well water.

          1. Us as well…so to speak. Went 600 feet, hit 2 more smaller veins meaning I didn’t need a cistern and pressure pump (fewer parts and pieces). When building drove up to continue on the foundation formwork, driller had a bit laying there, says “Just drilled the hardest 120 feet of granite ever.” Look at him, “Uh, do I have to pay for a new bit now?” (no, bit was 2 years old) Said we have great water…I can attest to that…Nectar of the Gods water underfoot….not to hard, too soft, just right….and zero government fiddling.

          2. there is little to no belief among the cognoscenti that brushing with a fluoridated toothpaste does any more to prevent decay/loss of dentition than brushing with a minty additive.
            to be effective fluoride should be incorporated into the dentin (roughly 70% hydroxyapatite);while the tooth buds are being developed in the fetus; again converting the hydroxyapatite into fluorapatite which has been shown to be more decay/acid/bacterial invasion resistant
            sorta like painting yourself with a water-soluble paint (to keep from getting wet) when it’s raining out

    3. The original studies over Fluoridated water were busted. The ‘control’ cities (non-fluoride enhanced water) started adding fluoride to their systems before the study was finished, so, no real correlation could be made. But people just jumped on the bandwagon.

      As to fluoride, curiously, the treatment for hyperthyroidism (too fast of a thyroid) is to treat it with small amounts of fluoride until thyroid functions are at the ‘normal’ level. In other words, fluoride kills the thyroid. Even more curious, hypothyroidism (‘slow’ or lowered thyroid function) has become rampant in America in areas where fluoridated water is common. But the brains assure us that there is no correlation between the rise of hypothyroidism and fluoride in the water. No correlation at all.

      The fluoride formulation used in toothpaste, which is what the studies about tooth decay were based on, is not the same formulation as used in water systems. The water-system fluoride really is an industrial waste product. Kind of like all those ‘additives’ that are in gasoline that are just waste products added in order to get rid of them.

      All of this has been known since, well, the time of the studies. But ‘experts’ and ‘the medical profession’ all know better than us and so ‘Shut up, peasant!’

  4. Medical insurance was fraudulent from the start. When HMOs were being addressed in Congress, it was obvious that they were set up to fleece the consumer. My sister was all for them since she lived just outside of D.C. and got the inside scoop telling her that they would reduce the health care costs for everyone and improve access to doctors. Years ago, counties had clinics for the indigent in their county and some doctors would charge based on your status. Less if you were on the poor side and a normal rate if you were middle class. Those county clinics are long gone. If I remember correctly the 9th Circuit Court directed that if a doctor had even one Medicare/Blue Cross/BlueShield patient the doctor had to charge all patients the same rate as the government rate on the whatever ails you spread sheet. (If you don’t know it, every little body malfunction has a medical code associated with it.) When the question about rising health care costs is raised, the answer is to get the government out of health care.

    Why did the Aussies build the Airtruk? Because PT-17s and other such planes used in crop dusting were worn out and a reliable replacement was needed. The Piper Pawnee was making inroads with US cropdusters, but the WW II biplanes were dirt cheap compared to buying a new Piper. Ag Cats were available but also expensive. The turbine powered Air Tractors were in the future. So in the typical manner of British influenced aviation design, the Airtruk was born. It filled a need in Australia but looks like some of the creatures from the outback.

    Back to drones. The AFU is using GPS targeting information provided by western countries to aid their continued attempts to overthrow Russia completely. What would happen if that data were not provided or GPS blocked for the region? Too bad about everyone caught in the artillery fire from either side.

    Syria is a mess. I fail to see the rejoicing expressed in the news or by online commenters, as while Assad was a harsh task master, Syria is now ruled by a known terrorist and will most likely start killing the Christians and opposing Islamic factions as soon as the reporters are back in their hotels. The big question is when will the gas pipelines be completed and who will get the cut from the pipeline fees and the Syrian natural gas. Will the pipelines be hooked up to Iraqi sources as was the goal years ago?

      Big insurers (and the govt is the biggest) set their own payments. This is expressed as a “discount” on the “rack rate”. So say we need to receive at least $250 to even cover the cost of doing a certain procedure. Now The Big Guys say, “we will pay you 25% of what you bill for, and you will like this because we are much bigger than you.” So we take it up the pooper on paper, but we just set the rack rate at $1200. Twenty-five percent of 1200 is 300, plus there’s the patient’s copay (say $30) so we actually make a miniscule profit. (Getting a net fee under $100 for doing some procedure is realistic, BTW.) Now, we’re gonna get stiffed by the patient in some cases, and we also treat the indigent at a loss (because of other stuff, not just the procedure), so it’s not as if we’re consistently making $50+$30 = $80 for each procedure done. BUT, the poor guy who’s running a tiny family business and doesn’t have insurance, but is not a leech on society, er, I meant underprivileged due to systemic racism and sexism and homophobia and anti-whatever, well, that working guy gets hit for the full $1200. And we (the medical system) will pursue that poor bastard for all of it. (Yes, increasingly there are advocate groups that help with fair billing, or you can maybe negotiate with the hospital billing department directly, but the “screwing the honest guy barely making it” is how the system is set up.) This is vicious, cruel, and bad for society. It is probably also plain evil, but I’m not a theologian and can’t comment on that.

      I’ve had patients who needed close follow up who told me flat out they wouldn’t (couldn’t) come back as often as required because of the co-pay. I tried to see if I could waive the co-pay. I was told by Billing, then Legal, that to do so would constitute fraud. Apparently we can’t bill Medicare/Medicaid if we don’t also pursue the co-pay. (This may or may not be true, it is what I was told.) On (admittedly rare) occasion I’ve paid the co-pay in cash out of my own pocket to get the patient to return. No, I was not being scammed. But obviously this is not sustainable, and I don’t particularly mean financially. The system is deeply screwed up. And that’s even not counting the mRNA vaxx nonsense and the “I AM the science!” assholes and so forth.

    6. Back in the 70’s when people first started noticing that medical costs were skyrocketing, it was discovered that the costs were due to Medicare/Medicaid. Government set the price it would reimburse for the procedure, said prices being set at NYFC and Washington DC levels, not Bugskuffle or Flyspeck prices.

      A few months ago I found a statuette of an old timey Chinaboy: lemon yellow skin, slitty eyes, shaved forehead and long queue in back. About 8” tall. Dressed in black robe/pajamas and skullcap. I keep forgetting to bring it to the office to display on my desk. I did show it to a waitress at some Chinese place (in Toronto where I found it). She said, “You look very happy!” By way of explanation I pulled the statuette out of the sack and said, “Yes, I just bought this.” She was puzzled. “You found a figurine of … a guy in historical costume?” She was FOB and failed to be offended by it. Turned out she didn’t even understand how offensive this thing is supposed to be according to modern sensibilities. Ahahaha!

Pretty sure you can’t buy those anymore as they’re probably “racciss”…we all thought it was cool. (Have a 15% off WM coupon…’purrito’ ornament is in the cart. Told the brothers and cousin I got a quantity discount on the Die Hard ornament…guess what they are all getting for Christmas! If I had everyone’s address you all would be getting one (especially our host, who I just know doesn’t have one adorning his wild grown/naturally harvested tree).

          2. Brer Rabbit: Joel Chandler Harris from “Uncle Remus: His Songs and Sayings” – a compilation of African-American – emphasize American – folk tales. Similar in nature to Aesop’s Fables. Keeps oral traditions alive but must be RayCyst because blacks are involved. “Song of the South” is a poor rendition of the tales but probably necessary to fit into movie format. Tar Baby is just one of the great tales.

  8. My grandfather was just the opposite, he favored the pistol over the rifle, because he felt if there was trouble, it was going to be up close, not at range. He was a RR engineer in the late 1800s early 1900s in the SW, and carried a SAA on the train, along with an 1886 Winchester to hunt antelope/deer from the train.

    I have Federal BC/BS medical as a retiree. I pay for it – seems to go up with higher COL increase than pension or SS COL – funny that, eh? I’m told I’m lucky to have this plan.

    Question: wifey wants to change. Says she can get something better cheaper. Without going into what should be … cause this is what is – should I change or keep the Fed insurance?

    Now why would I trust the answers on this site more than an insurance rep or a Fed bureaucrat? Self-answering question. :)

  12. Regarding “forced flouridation”.

    Perhaps it’s just me-

    Kari Ashauer: I don’t get to choose what’s in my water. I have no control over that, especially since it’s a state mandate they must put it in. Kentucky is one of 13 states that require water districts to add fluoride. Across the U.S., about 73 percent of people served by community water systems have fluoridated water.

    In Texas, beginning in the 70s, I used my own well water, certified flouride (and other things) free.
    In the 90s when I began homeschooling the “legacy”, at home and at the ranch, I continued using my own well water.
    My oldest homeschools his legacy (all 7 so far), and continues the tradition of having his own well.

    Just as I oppose woo flew mandates ( what does Black’s Law Dictionary say of “ mandates?), I steadfastly oppose other so called mandates ( tired old joke- am not talking mr mullato and byg myke). I also had/used various types of filtration systems for emergencies/backup.

    All of which makes me wonder if kari is in fact a wanna be
